Ambulatory surgery centers (ASCs) continue to expand the complexity of procedures performed in outpatient settings, including orthopedic, spine, vascular, and gynecologic operations with potentially significant blood loss. Although transfusion events in ASCs remain uncommon, perioperative teams must maintain readiness to recognize hemorrhage, stabilize patients, and coordinate rapid access to blood products when necessary. Current literature and regulatory guidance emphasize that transfusion preparedness in ASCs depends on careful patient selection, standardized protocols, and effective communication with nearby hospitals and blood banks. 

Most accrediting organizations and state regulations do not require ASCs to maintain a full in-house blood bank. However, facilities performing procedures associated with moderate or high bleeding risk are expected to have policies addressing emergency transfusion access, transfer agreements, and hemorrhage management. The Centers for Medicare & Medicaid Services (CMS), the Accreditation Association for Ambulatory Health Care (AAAHC), and The Joint Commission all require emergency preparedness systems capable of managing life-threatening complications, including hemorrhagic events. 

Preoperative screening is the cornerstone of transfusion readiness in outpatient surgery. Patients with anemia, coagulopathy, thrombocytopenia, anticoagulant therapy, or anticipated major blood loss require careful evaluation before ASC scheduling. Literature on perioperative blood management consistently demonstrates that optimization of hemoglobin levels and correction of reversible coagulation abnormalities reduce transfusion requirements and perioperative complications. Many ASCs use procedure-specific exclusion criteria to identify patients more appropriate for inpatient settings, particularly when estimated blood loss may exceed the resources of the facility. 

Communication with external blood suppliers and receiving hospitals is another critical component of preparedness. ASCs commonly establish formal agreements with nearby hospitals or blood banks to provide emergency blood products when required. These agreements should define transport logistics, anticipated delivery times, emergency release procedures, and contact pathways. In facilities located farther from hospital systems, the inability to rapidly obtain blood products may influence case selection and procedural limitations. 

Massive hemorrhage protocols adapted to the ASC environment have become increasingly important as outpatient surgery complexity grows. Although full protocols for massive transfusion are less common outside hospitals, ASCs should maintain algorithms for early recognition of bleeding, escalation of care, administration of intravenous fluids and vasopressors, activation of emergency medical services, and rapid transfer to higher-acuity facilities to ensure readiness in case of emergency. Simulation-based training has been shown to improve team coordination and crisis response during perioperative emergencies, including hemorrhagic events. 

Type and screen policies vary considerably among ASCs depending on procedural risk and institutional practice patterns. For low-risk ambulatory procedures, routine preoperative blood typing is often unnecessary. However, operations with higher anticipated blood loss may justify preoperative type and screen testing or arrangements for immediately available crossmatched blood at a partner hospital. Patient blood management principles increasingly encourage restrictive transfusion strategies and minimize unnecessary testing to reduce cost and resource utilization. 

Tranexamic acid and other blood conservation strategies can also reduce transfusion needs in ambulatory surgery. Orthopedic and spine literature demonstrates that antifibrinolytic therapy can significantly decrease blood loss and transfusion rates in appropriately selected patients. Additional measures such as meticulous surgical hemostasis, normothermia maintenance, and goal-directed fluid therapy further support blood conservation efforts in outpatient practice. 

Anesthesiologists play a central role in ASC blood transfusion readiness through preoperative assessment, intraoperative monitoring, and crisis management leadership. Early identification of patients at elevated hemorrhagic risk allows appropriate triage to inpatient facilities when necessary. During emergencies, anesthesia professionals coordinate resuscitation, hemodynamic stabilization, and communication with surgical teams and transfer centers. 

As ambulatory surgery continues to evolve, transfusion preparedness remains an essential patient safety priority. Effective readiness depends less on maintaining extensive on-site blood inventories and more on careful patient selection, standardized emergency protocols, interfacility coordination, and multidisciplinary training. Modern ASC blood management strategies emphasize prevention, rapid recognition, and efficient escalation of care to ensure safe outcomes in the outpatient environment.
